Faith Formation Launch on Wednesday, August 31, 2022

6:00-7:30pm in St. Mary Parish Hall

Parish Census Form (one per family)

Individual Student Registration Form (one per student)

Copy of Baptisim Certificate

Copy of Health Insurance Card

 Returning Students registering for Faith Formation 2022-2023

forms will be printed for you to proof, make changes and sign

at Faith Formation Launch on Wednesday, August 31, 2022

6:00-7:30pm in St. Mary Parish Hall

All Payments will be taken at Faith Formation Launch
